Insurance and Billing
Payment policy for office visits.
As a result of changes in health care, we no longer can extend credit
to our patients. Instead, we had to change to a strict payment policy.
Payment in full of the "patient's portion" is expected at time of service.
Patient's portion includes co-pays, unmet deductibles and office visits
and services not covered by insurance. For your convenience, we accept
cash, local personal checks, VISA and MasterCard.
If we participate with your insurance plan, you must pay the co-pay and
unmet deductible at time of service. If we do not participate with your
insurance, full payment for the office visit is expected at time of service.
Payment policy for contact lenses.
For newly fitted lenses a $100.00 deposit is due on the day of fitting.
Thereafter, one third of the balance is due each month. Replacement lenses
are payable in full at time of pick-up. Insurances are billed as a courtesy
to you and for patient reimbursement only.
Payment policy for in-hospital surgery.
We accept the amount approved by your insurance as payment in full. If
our charge is greater than the amount approved by your insurance, we will
not ask you to pay the difference. You are responsible for any co-pay
or unmet deductible mandated by your insurance.
Eligibility confirmation.
Confirm eligibility with your vision plan. It will prevent you from having
to pay out-of-pocket for services that normally would be covered by your
plan.
Referral and Authorization forms.
If your insurance requires that you obtain an authorization/referral form
from your primary doctor, you must do so before coming in. We are not
permitted to see you (except in some emergency situations) unless you
have an authorization. If you decide to pay personally for the visit,
please be aware that your insurance usually has the right to refuse to
reimburse you later, and they usually exercise this right.
Insurance participation and billing.
Insurance coverage policies change constantly. Because your insurance
coverage is a contract between you and the insurance company, the insurance
company does not notify us of every change. Therefore, we often cannot
determine if you are, or are not, covered for a specific visit or procedure.
To avoid surprises, and out-of-pocket expenses, please make sure to know
your coverage eligibility status.

If we participate with your insurance, we will submit the bill to the
insurance. If we do not participate with your insurance, we will provide
you with the necessary information for reimbursement.
